Plant and Equipment Prestart Checklist

Purpose. Completed by the operator before each shift for every item of powered mobile plant, elevating work platform, generator or compressor before it is used.
1.When to use this form
Complete a prestart check at the start of every shift for each item of plant you will operate, and again if the plant is handed to another operator. Do not operate plant that has not been checked. Do not operate plant you are not licensed or trained for.
2.How to complete it
Walk around the plant and check each item on the list. Tick the item if it is satisfactory. If any item is not satisfactory, do not use the plant, attach a Do Not Operate tag and report it to the site supervisor and the Yard and Plant Supervisor. Record the fault in the Plant and Equipment Register.
3.Critical items
A fault with brakes, steering, emergency stops, safety interlocks, seat belts, roll-over protection, guards, load charts, hydraulic hoses or the fire extinguisher means the plant must be taken out of service immediately.

- Seat belt, roll-over and falling object protection
- Guards, covers and warning decals
- Hydraulic hoses, leaks and tyre condition
- Horn, lights, reversing alarm and mirrors
- Load chart, plant sticker and service date
4.Records
Keep the completed checklist with the plant or in the site office for the duration of the project. The Yard and Plant Supervisor reviews prestart records during monthly plant inspections.
5.Hired plant
Hired plant must be checked the same way. If hired plant arrives damaged or without a current service sticker, record it on this form and tell the Yard and Plant Supervisor before use so the hire company can be notified.
