The situation

What has happened

The 15 metre diesel boom lift KP-035 failed its prestart check this morning with a hydraulic leak at the boom cylinder. It is tagged Do Not Operate in the yard. The Bayview Early Learning Centre site has the boom lift booked for tomorrow to install fascia and gutters on the two-storey section, and the site supervisor is asking for a unit by 7:00 am. The plant register shows the 10 metre scissor lift is on another site and the skid steer is overdue for service. Hire companies can supply a boom lift at short notice but the operator must hold the right licence and the site has a delivery window restricted by the school zone. The Yard and Plant Supervisor wants the register kept accurate throughout.

Evidence guide

Look for a correctly completed prestart form with the Do Not Operate outcome, register entries that are accurate and dated, recognition that a boom lift over 11 metres needs a high-risk work licence and not just a yellow card, and a delivery booked outside the school zone window.
